The Eon Patient Navigator ("EPN") is responsible for reviewing incoming radiology reports and registering them into the Eon software application along with the expected follow-up exam as per the clinical guideline decision aid protocols. The EPN is an integral and key member in the Clinical Transformation department who ensures appropriate longitudinal tracking and evidence based follow up via our Care Management solution. Care Management (CM) is a new, radically different approach to the longitudinal management of patients enrolled in screening programs or patients who present with an incidental finding that needs tracking. 

What You Do:
  • Review incoming radiology reports and identify any incidental findings
  • Review incoming radiology reports and identify if the patient is in a screening program
  • Enter next appropriate follow-up for patient as outlined in the clinical guideline decision aids
  • Manage high-risk patients in CM managed Coordinator Review work list
  • Assist in CM Quality & Improvement processes
  • Participate in peer to peer mentoring
  • Participate in on-going cohort education and growth
  • Attend all departmental and/or organizational meetings as required
Knowledge & Skills:
  • Associate degree or higher in a Medical field such as RN, RRT, RT
  • Required foundational knowledge base in medical terminology, anatomy, and physiology
  • Background in oncology related field preferred
  • Oncology navigation related experience preferred
  • Proficiency with Apple MacBook preferred
  • Proficiency with Google Suite preferred
  • Ability to work in a high stress environment
  • Ability to apply critical thinking to make clinical decisions
  • Ability to think quickly
  • Ability to multitask

This is an hourly, non-exempt role.  The annualized pay range for this position in Colorado is $50,000 - $65,000 (yr base pay); however, base pay offered may vary depending on job-related knowledge, skills, and experience.  This information is provided per the Colorado Equal Pay Act. Base (CEPA) pay information is based on market location.
